HCA Healthcare Inc
HCA Healthcare owns and runs hospitals, emergency rooms, surgery centers, and other medical facilities, mostly in the United States and the United Kingdom through its joint venture. It gives patients the places and staff needed for inpatient care, outpatient procedures, childbirth, trauma care, and many routine and specialized treatments. It also employs or works closely with doctors, nurses, and clinical teams to deliver that care. Its customers are patients, but most of the money flows through health insurers, employers, Medicare, Medicaid, and other payors that reimburse the care delivered in HCA facilities. HCA makes money by billing for hospital stays, surgeries, emergency visits, doctor services, and related healthcare services. In simple terms, it is a large hospital operator that gets paid every time it treats patients under these payment systems. What makes HCA’s business different is that it sits at the center of local healthcare delivery. It does not sell a single drug or device; it runs the physical sites where care happens and coordinates a wide range of services around them. That makes it an important middle layer in the healthcare system, connecting patients, doctors, and payors in the day-to-day delivery of medical care.

Guidance cut: HCA lowered full-year 2026 guidance after the first half showed a much bigger-than-expected shift from exchange-covered patients to uninsured patients.
Volume held up: Even with the payer mix pressure, same-facility admissions rose 2.5% and ER visits rose 3.6% in the quarter, with insured volumes excluding exchanges still growing.
Exchange headwind: Management now believes almost all exchange coverage losses are becoming uninsured, instead of the 80% to 85% assumption used earlier.
Cost discipline: The company said its resiliency program helped keep same-facility costs essentially flat versus last year after Medicaid supplemental payments.
Capital plan: HCA kept its $5 billion to $5.5 billion CapEx plan and said it has approved more than $7 billion of projects that should come online over the next 3 years.
