HCA Healthcare Inc
HCA Healthcare owns and runs hospitals, emergency rooms, surgery centers, and other medical facilities, mostly in the United States and the United Kingdom through its joint venture. It gives patients the places and staff needed for inpatient care, outpatient procedures, childbirth, trauma care, and many routine and specialized treatments. It also employs or works closely with doctors, nurses, and clinical teams to deliver that care. Its customers are patients, but most of the money flows through health insurers, employers, Medicare, Medicaid, and other payors that reimburse the care delivered in HCA facilities. HCA makes money by billing for hospital stays, surgeries, emergency visits, doctor services, and related healthcare services. In simple terms, it is a large hospital operator that gets paid every time it treats patients under these payment systems. What makes HCA’s business different is that it sits at the center of local healthcare delivery. It does not sell a single drug or device; it runs the physical sites where care happens and coordinates a wide range of services around them. That makes it an important middle layer in the healthcare system, connecting patients, doctors, and payors in the day-to-day delivery of medical care.

Quarterly results: HCA said first-quarter revenue rose 4.3%, adjusted EBITDA increased almost 2%, and adjusted diluted EPS increased approximately 11% versus last year.
Volume headwinds: A much milder respiratory season and January winter storms reduced volumes and hurt EBITDA by an estimated $180 million, but management called those effects temporary.
Supplemental payments: Medicaid supplemental programs were better than expected, adding about $200 million to adjusted EBITDA versus the prior quarter and partly offsetting the volume shortfall.
Guidance unchanged: HCA reaffirmed its full-year 2026 guidance, saying the first-quarter disruption did not change its view of volumes, payer mix or costs for the rest of the year.
Exchange pressure: The company estimated a $150 million first-quarter EBITDA impact from exchange-related coverage changes and still thinks the full-year impact will be $600 million to $900 million.
Operational themes: Management highlighted continued progress in digital transformation, AI, quality, patient satisfaction and network expansion, while also noting elevated payer denials and underpayments.
